Dear Breakthrough Families,

As we begin this unprecedented school year, we recognize that it will not be without challenges and uncertainties. However,we are committed to working closely with the City of Cleveland Department of Public Health to limit the spread of COVID-19 in the community, and we believe that together we can all work to make this a safe and rewarding year for everyone. 

With this in mind, beginning immediately, we are asking families to report within 24-hours all cases of scholars who test positive for COVID-19 OR who are diagnosed with COVID-19 by a healthcare professional. Please call your child’s school or complete the information in this form, even if your child is learning remotely and has not been present in any Breakthrough buildings

When you call the school, please be prepared to share your scholar’s name; grade; school; date they were tested; and date you received the positive test. Please also be prepared to share if your child has been physically present in any Breakthrough Schools building within 10 days of receiving a positive test.

Your cooperation will enable us to comply with Ohio Department of Health orders to track and report positive cases of COVID-19 in our school community.
